2 listopada 2021 r. Operator Programu Zdrowie podpisał z Fundacją "Instytut Profilaktyki Zakażeń" umowę w sprawie przyznania finansowania na realizację predefiniowanej inicjatywy bilateralnej pn. „Zmniejszenie dysproporcji zdrowotnych w programach szczepień i racjonalnym stosowaniu antybiotyków w ambulatoryjnej opiece zdrowotnej w Polsce”. Inicjatywa finansowana jest ze środków MF EOG i NMF 2014-2021 w ramach Funduszu Współpracy Dwustronnej.
Beneficjentem inicjatywy jest Fundacja "Instytut Profilaktyki Zakażeń", natomiast ze strony norweskiej realizację zadań powierzono Norweskiemu Dyrektoriatowi ds. Zdrowia oraz Norweskiemu Instytutowi ds. Zdrowia Publicznego.
Całkowity budżet inicjatywy wynosi 110 500,00 euro, co stanowi równowartość 494 609,05 zł. Finansowanie dzielone jest na dwie części: 49,15%, tj. maksymalnie 54 310,75 euro pochodzi z Mechanizmu Finansowego EOG 2014-2021 oraz 50,85%, tj. maksymalnie 56 189,25 euro pochodzi z Norweskiego Mechanizmu Finansowego 2014-2021.
Celem Inicjatywy FWD jest zmniejszenie nierówności zdrowotnych poprzez poprawę dostępu do profilaktyki i leczenia chorób zakaźnych w oparciu o nowoczesne narzędzia i aktualną wiedzę medyczną oraz poprawa bezpieczeństwa i jakości opieki zdrowotnej poprzez optymalizację realizacji programu szczepień i terapii zakażeń, z uwzględnieniem zmniejszenia ryzyka oporności na kluczowe antybiotyki.
W odniesieniu do dziedziny szczepień ochronnych inicjatywa FWD będzie się skupiać na podnoszeniu świadomości i tworzeniu dostępu do wiedzy personelu medycznego w dziedzinie zapobiegania chorobom zakaźnym poprzez działania edukacyjne, z uwzględnieniem skutków pandemii COVID-19 i czerpania korzyści z obecnego tempa rozwoju społeczeństw w zakresie zapobiegania chorobom zakaźnym. Planowana jest również optymalizacja realizacji programu szczepień ochronnych poprzez wprowadzenie nowoczesnych narzędzi wspomagających decyzje personelu medycznego.
Inicjatywa podzielona jest na trzy części:
1. Działania w zakresie szczepień ochronnych:
a) opracowanie wstępnego sprawozdania na temat wpływu pandemii na Krajowy Program Szczepień oraz analizy obecnej sytuacji na podstawie badania opinii personelu medycznego;
b) opracowanie analizy wyników badań i opracowanie sprawozdania, z uwzględnieniem trudności w dostępie do nowoczesnych narzędzi, wpływu pandemii na wdrażanie szczepień ochronnych, potrzeb oraz wymagań merytorycznych i technologicznych opracowanego narzędzia;
c) opracowanie i wdrożenie algorytmu wspierającego wdrażanie rutynowych szczepień przeciwko 14 chorobom zakaźnym biorąc pod uwagę kwalifikacje do szczepienia, identyfikację przeciwwskazań do szczepień, uzupełnienie opóźnionych i przerwanych szczepień;
d) opracowanie i wdrożenie modelu narzędzia informatycznego ułatwiającego wykorzystanie algorytmu i aktualnej wiedzy w dziedzinie szczepień;
e) testowanie i pilotażowe wdrażanie narzędzia informatycznego w dwóch wybranych placówkach podstawowej opieki zdrowotnej w obu regionach oraz ocena jego przydatności w codziennej praktyce.
2. Działania w zakresie antybiotykoterapii:
a) opracowanie analizy obecnej sytuacji dot. antybiotykoterapii na podstawie badania opinii personelu medycznego;
b) opracowanie analizy wyników badań i sprawozdania z uwzględnieniem trudności w dostępie do nowoczesnych narzędzi, wpływu pandemii na wdrażanie ostrożnych stosowania antybiotyków, luk w wiedzy oraz wymogów merytorycznych i technologicznych;
c) dostosowanie i wdrożenie planu działania w zakresie wdrażania istniejących wytycznych dotyczących stosowania antybiotycznego w praktyce ogólnej, wspierającego wybór leczenia antybiotykami, z uwzględnieniem zmniejszenia ryzyka oporności na antybiotyki;
d) projektowanie i opracowywanie i wdrożenie modelu narzędzia informatycznego;
e) testowanie i pilotażowe wdrażanie narzędzia informatycznego w dwóch wybranych klinikach podstawowej opieki zdrowotnej w obu regionach oraz ocena jego przydatności w codziennej praktyce;
3. Szkolenie i rozpowszechnianie opracowanych narzędzi i ocena projektu:
a) opracowanie i administrowanie stroną internetową poświęconą projektowi;
b) zarządzanie projektem;
c) szkolenie personelu medycznego w celu podniesienia poziomu praktycznej wiedzy w dziedzinie szczepień ochronnych i ostrożnego stosowania antybiotyków;
d) ocena szkoleń i narzędzi informatycznych, a także propozycje poprawy na podstawie ankiety oraz przeniesienie majątkowych praw autorskich i zależnych do tych dokumentów;
e) upowszechnianie wyników uzyskanych podczas realizacji projektu i wdrażania opracowanych narzędzi.
Kwalifikowalność wydatków rozpoczęła się od dnia wydania decyzji przez Komitet Współpracy w ramach Programu Zdrowie tj. 20 listopada 2020 r. i kończy się w dniu 30 czerwca 2022 r.

On November 2, 2021, the Programme Operator for Programme Health signed an agreement with the Foundation for Infection Prevention Institute on granting funding for the implementation of a predefined bilateral initiative entitled "Reducing health disparity in immunization programs and prudent use of antibiotics in Polish outpatient health care." The initiative is financed by the EEA and NFM 2014-2021 funds under the Fund for Bilateral Relations.
The initiative’s promoter is the Foundation for Infection Prevention Institute, while on the Norwegian side, tasks were entrusted to the Norwegian Directorate of Health and the Norwegian Institute for Public Health.
The total budget of the initiative is EUR 110,500.00, which is the equivalent of PLN 494,609.05. The financing is divided into two parts: 49.15%, i.e. a maximum of EUR 54 310.75 comes from the EEA Financial Mechanism 2014-2021 and 50.85%, i.e. a maximum of EUR 56 189.25 comes from the Norwegian Financial Mechanism 2014-2021.
The goal of the FBR initiative is to reduce health inequalities by improving access to prevention and treatment of infectious diseases based on modern tools and current medical knowledge and to improve the safety and quality of health care by optimizing the implementation of the vaccination and infection therapy program, taking into account the reduction of the risk of resistance to key antibiotics.
Regarding the field of immunization, the initiative will focus on raising awareness and creating access to healthcare professionals' knowledge in the field of communicable disease prevention through educational activities, taking into account the effects of the COVID-19 pandemic and benefiting from the current pace of development of societies in the field of communicable disease prevention. It is also planned to optimize the implementation of the preventive vaccination program by introducing modern tools to support the decisions of medical personnel.
The initiative is divided into three parts:
1. Actions in the field of preventive vaccinations:
a) preparation of a preliminary report on the impact of the pandemic on the National Immunization Programme and an analysis of the current situation based on a survey of medical staff;
b) preparation of the analysis of research results and preparation of a report, taking into account difficulties in accessing modern tools, the impact of the pandemic on the implementation of preventive vaccinations, the needs and substantive and technological requirements of the developed tool;
c) development and implementation of an algorithm supporting the implementation of routine vaccinations against 14 infectious diseases, taking into account vaccination qualifications, identification of contraindications to vaccinations, supplementing delayed and interrupted vaccinations;
d) development and implementation of an IT tool model to facilitate the use of the algorithm and current knowledge in the field of vaccination;
e) testing and pilot implementation of the IT tool in two selected primary health care facilities in both regions and assessment of its usefulness in everyday practice.
2. Activities in the field of antibiotic therapy:
a) preparation of an analysis of the current situation regarding antibiotic therapy based on a medical staff survey;
b) preparation of an analysis of research results and a report, taking into account the difficulties in accessing modern tools, the impact of the pandemic on the implementation of prudent use of antibiotics, knowledge gaps, and substantive and technological requirements;
c) adaptation and implementation of an action plan for the implementation of existing guidelines for antibiotic use in general practice, supporting the choice of antibiotic treatment, taking into account the reduction of the risk of antibiotic resistance;
d) design, development and implementation of an IT tool model;
e) testing and pilot implementation of the IT tool in two selected primary health care clinics in both regions and assessment of its usefulness in everyday practice;
3. Training and dissemination of developed tools and project evaluation:
a) development and administration of a website dedicated to the initiative;
b) initiative management;
c) training of medical personnel in order to increase the level of practical knowledge in the field of immunization and the careful use of antibiotics;
d) evaluation of training and IT tools, as well as suggestions for improvement based on the survey and transfer of proprietary copyrights and derivative rights to these documents;
e) disseminating the results obtained during the project implementation and implementation of the developed tools.
The eligibility of expenditure started on the date of the decision taken by the Cooperation Committee under the Programme Health, i.e. on November 20, 2020 and will end on June 30, 2022.
